Got the codex entry from one of the Rakghouls in the sliding plate part of Sinking City.
I unlocked this codex entry upon killing the Scavenging Rakghoul (big red guy) at roughly 500, -500, in the Sinking City/Sliding Plate.
However, this kill coincided with me fulfilling the bonus mission for Sergeant Mack’s “Suicide Mission”, entitled “Blaster Fodder” — so the possibility exists that it’s because of the mission, and not the kill, that I got this codex unlock. However, in my experience, TOR doesn’t make codex entries a one-time-only affair — which is what this entry would be, if it’s tied to an optional, unrepeatable bonus mission.
I found a Lumbering Rakghoul at -1532, -184 that gave me the Codex for them.
I think Treggy was using (x,z) when posting. As there are no lumbering rakghouls where he said.
At least for Empire, only the Lumbering Rakghoul gives the codex. Groups of them can be found at x: -1516, y: 160 and x: -1471, y: 205.

Killed without Quest,got Codex Entry. Republic.
I got the codex entry from a Scavenging Rakghoul strong at 499, -496, -151 in the Sliding Plate area as Republic, without a quest.
x= 345 y= -219 (at the sinking city, upper part) Killed a big red Rakghoul near a jedy statue and got the codex, wasnt doing any mission.
