I found this lore object in the “Temple of Sacrifice” operation on Yavin 4. It is a very small item called “lost datapad” on the ground.
You can get while you are on your way to the second boss. The lore object is behind 3 Massassi, you have to kill them to get it.
The coordinates are (-335,-15)
