This entry is earned on Alderaan for choosing to kill all of House Girard when given the option. It is Empire only to my knowledge, and I completed it with my Bounty Hunter.
Technically, to achieve this title you have to accept the quest titled Silver Tongues, which gives you the option to return to House Girard to kill everybody. The mission then morphs to Kingmaker For A Day quest. After going to house Organa, you return to House Girard. Upon killing everybody in the house in a cut-scene, you are instructed to return to your Hanger, then you have to return to your ship and use the Holoterminal. THEN you actually get the codex title. This was on my Bounty Hunter.
