This is an actual Planet entry that you can find under “Planets” and not “Locations”. You obtain it once you travel to CZ-198 in the Unknown Regions in your ship.
I’ve used both priority transport and galaxy map many times to enter CZ-198, but no luck and no codex for me.

What a pack of lies! This entry isn’t under Planets but Locations as swtor spy correctly stated.
In order to get this entry, one need to use their macrobinoculars and scan the bridge between the Twin Towers at the center of CZ-198 map.
Locations – CZ-198 now available
cz-198, -547, 40 (for imp you need others coordinate)
You need to use your macrobinoculars to scan the Twin Towers to the south of the CZ-198 map

For the Empire you need to scan the twin tower like on rep side.
You can see the tower from the mission terminal.
Aim for the crossing bridge between the two towers. You have to be in the exact right place to see it. Took a few times for the scan to activate and register.
I found best spot for me was
– 547, 29, 88
Left click on connecting bridge (like someone else mentioned)
