Actually it’s not – I’ve opened this entry by finnishing quest arch that ended in The King’s Hall and fight with Gormak king Jokull 😉
I thought Jokull in the quest The Gormak King but haven’t received the entry (empire side). Seems to be bugged.
For Republic : there was a patch that fixed it. If you don’t have it, go redo “Primary Targets” (seems you are allowed to do this, maybe for the fix).
I received this codex (Republic Side) after defeating Jokull in the quest Primary Target in the Kings Hall (entrance approx 150, 2360) of the Gormak Lands. I picked up the quest approximately 50,1525.

I’ve received this on multiple characters for Republic, but not for empire. Appears to still be bugged for empire.
Hi, well its still bugged for both pub and emp. I have 1 char: Commando lvl 60 still missing the codex and cant get it. And empire chars missing it too..
